Erectile dysfunction, or ED, affects a significant portion of men globally, ranging from 5% to 20%. Historically, treatment has often centered around medications like PDE5 inhibitors, which don’t work for everyone. Recently, there’s been intriguing research into using botulinum neurotoxin, commonly known as Botox, as a potential treatment.

A recent study investigated if shear wave elastosonography (SWE)—an ultrasound technique to measure tissue stiffness—could predict the success of Botox injections for ED treatment. The study involved 20 men aged between 18 and 70 who hadn’t responded to PDE5 inhibitors.

Each participant received Botox injections directly into the corpus cavernosum, combining it with measurements from SWE before and after using intracavernosal prostaglandin E1 (PGE1). The effectiveness was gauged using three key metrics: the International Index of Erectile Function (IIEF-5), the Sexual Encounter Profile (SEP), and the Erection Hardness Score (EHS).

The results were promising. Participants showed marked improvement in erectile function. In essence, this study suggests that Botox could be a viable option for those not responding to traditional ED medications, and SWE might help predict who could benefit the most from this treatment.
